  • Press 2 to have a medical plan benefit profile faxed to you
  • Press 3 to have a dental plan benefit profile faxed to you
  • Press 4 to have a benefit profile mailed to you
  • Press 5 for year-to-date (YTD) out of pocket and deductible information
  • Press 7 for general information
