    Provider Resources: IMA allows Providers to check claims status or access medical and dental plan benefit profiles in one of several ways:


    Responses to Inquiries/Hours: All requests for assistance are processed Monday through Thursday, 9AM - 5PM and Friday 9AM - 3PM Central Time. Whenever possible, IMA will respond to all requests within one business day.


    Verification of Benefits: Enrollees and providers may request a Verification of Benefits summarizing current coverages.

    Verifications of Benefits contain the following information:

  • Primary Employee's name, date of birth and coverage dates
  • Current Plan Coverages
  • All covered dependents, their names, gender and birthdays (if applicable the date dependent's coverage terminated).
  • Current Plan Deductible limits
  • Deductible met to date
  • Coinsurance rate
  • Out-of-Pocket limits
  • Note that Current Out-of-Pocket totals may be requested via the Benefits Inquiry form or by using the Interactive Voice Response System (IVR)
